- Look in the email inbox associated with your Little Otter account for an email with the subject “[Provider Name] at Little Otter has sent you a form”
- Please check your spam folder as well
- This email will contain a link to the form and a “password” that must be entered after clicking on the link.
- If you cannot locate the email, please contact the provider who sent it to you or help@littleotterhealth.com and we would be happy to resend it to you.
